D4 Medical in Reading and London

We provide fully registered GMC doctors to complete the medical examination report for your lorry or bus driving licence DVLA D4 form.

We do not routinely complete the D2 form, which you will need to complete yourself. Our medical assessment costs £60 which includes an eye test.

The examination should take roughly 15 minutes to complete.

We require you to bring the following additional documents to your appointment:

• Driving licence
• Your driving glasses, you must bring your glasses prescription (do not wear contact lenses as these will need to be removed for the eye test)
• Any prescriptions for current and regular medications
• A list of all your current medication or bring all the medication boxes
• Any hospital letters or details of hospital consultants who are treating you

The D4 form requires very detailed medical information from you so it helps to please bring as much information as possible.

What to expect during the medical assessment:

• One of our friendly doctors will take a medical history to comply with the D4 form requirements
• Record basic health information, such as height and weight
• Record a Blood pressure measurement
• Perform an eye test (one test without and one with corrective glasses if worn)

Please do not wear contact lenses as these will need to be removed for the eye test. The eye test is included in our medical fee.
