D4 FAQs

The medical will usually take between 10 - 15 minutes.

If you have any medical conditions we may require a printed copy of your medical summary to complete the D4 medical. This can be provided by your GP practice upon request and is there is no charge for them providing this information.

If you wear corrective lenses we will need to know the strength of the lenses.

Yes. However, in the event you are unable to make the vision standard we will ask you to complete this section with an optician and only complete the medical section of the form.

Potentially. If you have any underlying medical conditions we may need access to your medical summary to complete the form. Otherwise there is a risk we will be unable to complete your medical in that appointment and may require you to re-book which will inevitably delay your application.

Yes. In accordance with government Covid-19 regulations we require all applicants to wear a face mask covering.

Ideally yes. Your appointment will be quicker if you can begin to fill out your own details on the front page of the D4 form and your name and DOB on the bottom of each subsequent page.

We will return the forms to you. You will need to self complete the D2 form and post both forms to the DVLA to process.

On a weekly basis, often in the evenings and weekends for added convenience.

Our D4 Medical costs £60, which is substantially cheaper then then standard GP fee.

Taxi FAQs

It will depend on your governing council.

Reading council allows any registered doctor to complete your taxi medical.

Some councils specify only registered Gp undertake your medical.

Some councils require the performing doctor to have access to your medical records.

We recommend you check the rules with your local council to clarify their requirement before booking.
